ThedaCare has named Carrie Riley to the newly created position of director of population health development and integration. Riley will lead ThedaCare’s efforts to research, explore and implement health care models that serve certain patient groups.
By September, pilot programs for a group of patients in Shawano and ThedaCare Physicians-Internal Medicine in Appleton will be in place to help address the unique needs of these individuals.
“As part of our research, we interviewed patients with layers of health and psychosocial challenges and learned what is not working for them in a traditional care model,” Riley said.
Such patient groups, or populations, help to reformulate care teams and care delivery methods to meet their existing conditions and rising risks.
“Our goal is to help people live more comfortable and healthier lives while responding to the changing health care landscape,” Riley said. “We must be innovative and agile to remain sustainable and effective. It’s our job to seek radically new ways to solve problems to help patients who need the most care.”
Riley joined ThedaCare in 1986 and most recently served as the practice administrator for ThedaCare Physicians’ Internal Medicine, Endocrinology, and Infectious Disease clinics. Prior, she was a quality manager for Physician Services, Senior Services, Home Care and Behavioral Health.
ThedaCare is a community health system consisting of five hospitals: Appleton Medical Center, Theda Clark Medical Center, ThedaCare Medical Center-New London, Shawano Medical Center, and Riverside Medical Center in Waupaca. ThedaCare also includes ThedaCare Physicians, ThedaCare Behavioral Health, ThedaCare At Work, and ThedaCare At Home.
