In an effort to streamline services and maximize efficiencies for patients, a number of changes will be made to the HSHS St. Clare Memorial Hospital Prevea Oconto Health Center and the Prevea Oconto Health Center.
HSHS St. Clare Memorial Hospital Prevea Oconto Health Center is located at 103 First Street, and the Prevea Oconto Health Center is located at 620 Smith Ave. On April 30, the First Street location will close and relocate all services to Smith Avenue.
The First Street location offers family medicine and prescription services. Beginning May 4, physician assistant Rob Birk, the sole provider of family medicine services on First Street, will see patients at the Smith Avenue location. Appointments can be made by calling 920-834-4110.
Prescription services will also move to the Smith Avenue location on May 4. Prescription services offer both prescriptions and over-the-counter items to patients. Once added, it will be a new service offered for patients who visit the Prevea Oconto Health Center. With the move to the Smith Avenue location, the phone number for prescription services will change to 920-848-6336.
On May 2, urgent care services at Smith Avenue will no longer be available. Those in need of urgent care services can find care at the HSHS St. Clare Memorial Hospital Prevea Oconto Falls Health Center at 835 Main St., Oconto Falls. Hours are 8 a.m. to 8 p.m. Monday through Friday and 8 a.m. to 4 p.m. Saturdays, Sundays and holidays.
Emergency care is also available in the same location in the emergency department of HSHS St. Clare Memorial Hospital in Oconto Falls.
