Care Wisconsin will begin serving members in Shawano County who have transitioned from the previous county-administered long-term care programs Sept. 1.
This is the first time the Family Care program has been offered in Shawano County. Family Care is a government-funded program that helps frail seniors and adults with disabilities manage their home and community-based long-term care services.
The Family Care program is also being offered in Brown, Door, Kewaunee and Oconto counties, with members in Marinette and Menominee counties transitioning in the coming months.
Members are central to a care team that includes the member, a care manager and registered nurse. Together they create a plan that meets the member’s long-term care needs based on the member’s goals, needs and abilities.
“We have seen countless members benefit in so many ways from the Family Care program – from more independence to improved quality of life,” said Karen Musser, Care Wisconsin CEO. “We are extremely excited to see this program move into these counties.”
Care Wisconsin contracts with local agencies and providers to serve Family Care members and offer them flexibility and access to high-quality services for their long-term care needs. In addition to securing partnerships with local providers, Care Wisconsin has been focusing on developing relationships and working collaboratively with other key stakeholders in these counties. Staff has met with representatives from the county, aging and disability resource centers, providers, other local organizations and interested individuals over the past months.
Additional members in Shawano County will continue to transition into the program through December.
Care Wisconsin has an office at 300 East Green Bay St. in Shawano. There is also a regional office at 2321 San Luis Place, Green Bay, as well as offices in Door and Marinette counties.

Care Wisconsin has extensive experience delivering community-based health and long-term care services and currently serves more than 8,100 members in 32 counties.
With roots as a human services agency, Care Wisconsin was founded in 1976, starting the region’s first adult day center in a church basement. Today, Care Wisconsin offers three public programs: Family Care, Partnership and Medicaid SSI.
