ThedaCare conducting health and well-being survey

With a mission of improving community health, ThedaCare is dedicated to understanding each person’s unique medical background and goals, as well as social factors.

As a commitment to that mission, ThedaCare has launched a survey to understand overall health and well-being, particularly as it relates to the impacts of the COVID-19 pandemic.

The survey is open to all community members living or working in Shawano and Menominee counties. The survey can be accessed at thedacaresurvey.com, and is open until May 14.

“ThedaCare is committed to empowering our patients and communities with programs and services to improve their health and well-being,” said Dr. Mark Cockley, ThedaCare chief clinical officer and president of the clinically integrated network. “To help develop those resources, we want to hear from community members to truly understand the needs in our area, develop solutions and inspire action that leads to better health.”

Through the survey, individuals are encouraged to share how they’re personally feeling now, and to look ahead to the next five years, related to physical, mental and financial health, as well as social and emotional support and life purpose and fulfillment. Feedback about overall community well-being also will be sought.

The survey takes about five minutes to complete, and individual responses will be kept confidential. Results will be available late this summer.
