Unity, northeast Wisconsin’s first nonprofit hospice and palliative care provider, has partnered with Meadowbrook at Oconto Falls, to open a dedicated hospice wing.
The collaboration enables Unity and Meadowbrook to meet the growing demand for hospice care delivery.
The Unity Sunset Wing at Meadowbrook, located at 100 E Highland Drive, Oconto Falls, provides a homelike, caring environment for around-the-clock hospice care until the end-of-life, as well as for temporary caregiver relief.
Patients and families can benefit from a full complement of compassionate staff who provide medical, emotional, spiritual and grief support, as well as amenities such as private and semi-private rooms, freshly prepared meals and a family gathering area. Visits from family, friends and pets are welcome at any time.
Members of the public are invited to attend a ribbon cutting celebration June 30. A ribbon cutting presentation will begin promptly at 10 a.m., followed by light refreshments and tours.
“We are thrilled to be partnering with Meadowbrook. Collaboration between nursing homes and hospice is essential to improve access to quality care for more patients during end-of-life,” said Alisa Gerke, Unity Hospice executive director. “The hospice wing is a reflection of our unwavering commitment as a nonprofit hospice to respond to the needs of our communities and put people over profits.”
“While residents continue to receive quality care and services from Meadowbrook staff, our partnership with Unity enables our hospice patients and families to receive an extra layer of personalized support from Unity nurses, social workers, chaplains and grief counselors,” said Chris Pryor, Synergy senior care chief marketing officer. “Hospice services offered by Unity are fully covered by Medicare and most insurance plans.”
